It looks like it is one of the updates. I have some users seeing this as well. The workaround is clicking cancel and then clicking on the "Need password" prompt in the lower right, but it still prompts for the smart card. Apparently, uninstalling a recent update installed today will fix this. I am going to try it out as soon as I can. Yeah I have removed all the new updates and same problem. Have to wonder if Microsoft will supply a hotfix. I may give them a call tomorrow.
I hate the idea of installing office again because of an update. Ignoring the cert results in being unable to save the password in Outlook clients when using Outlook Anywhere. So if you need that ignoring the cert request is not an option. A fix is still required. Vitaltech Consulting, Inc.

I ignore it and it keeps prompting for their login and it wont take. I think it also has something to do with Autodiscover, but I have no idea what it is. I've removed the KB update and rebooted and removed it from the download list. But still nothing. Same thing. KB was not installed on my computer - I have the same pop up Windows security - "insert a smart card". Waiting to see if it returns but thought it was interesting that last year it was a hotfix that changed some of the SSL settings causing the same error I created for myself.
Ended up using the custom error page to redirect per the link I posted with the defaults - less intensive process and easier to implement!

Are they trying to electronically sign something when it comes up, I know Adobe can behave this same way if someone is trying to sign a document. Try saving a copy of the document to another format, like the older. DOC instead of. If there are any messed up formatting codes in the document, they should be stripped out when saving to a format that doesnt support them. Is there a reference or link in the document image, linked file, UNC path reference, etc.
If these particular documents came from outside your organization, such references may be trying to access something on the network where the documents originated. There is one reference that links to a program called "TAM" through an add-in, but it is always successful at pulling the string client name into the doc. Both the Word doc and the TAM server are local. Something that might be pertinent that was just mentioned to me by the user: If she opens this doc from the drive it resides on or in an emailshe gets the error.

In Device Manager the reader shows up when connected.I am trying to use the below commands to repair a cert so that it has a private key attached to it. When I run the command it brings up the authentication issue, but will only let me choose "Connect a Smart Card. Any ideas why it is not letting me type in a password? On the TechNet forums, I found that I had to export the cert again from my current Exchange server that I need it on, and exported it with a private key this time.
This should add the cert with the private key. Normally, certs won't show up in Exchange and other places when you have the cert installed but missing the private key. I broke down and called MS. Called in on Friday, and didn't get help till 2am Tuesday Morning. My tech disappeared and they wouldn't assign a new one till I demanded a manager and sat on the phone waiting for hours. Long day.
Anyway, the tech couldn't figure out why the cert was coming from godaddy without the key, nor why the certutil was not working. Couldn't get past the smart card prompt. What he did was show me how to use the mmc to re-key the cert.
Basically took the info from the cert, then deleted from the mmc. Then created the new text file and I sent to godaddy. When it was done first we imported the cert to personal. It didn't show up with a key.

Then imported the GoDaddy root to the Trusted root cert folder. Then the key appeared. The system will ask for the smart card when it can't connect with your active directory. All DNS and domain settings are correct. I don't have any issues with logging in as the domain admin or resolving any server names on the network.
I use some LDAP, but nothing running on this server. Exchange has been installed, but no setup yet. I have changed server name and joined to the domain.
